What to Expect
Your first appointment will be arranged by our friendly front desk team. Once it’s scheduled, you’ll receive a link for a virtual meeting with a treatment coordinator. Your referral, medical history and any images that are shared will be evaluated by Dr. Holtzen, and he will develop a preliminary treatment plan. During your virtual Intake Interview, you’ll dive into the details of what to expect during the treatment, discuss options for treatment, review medical history, get information about insurance matters, discuss out of pocket expenses, and be able to ask any questions you have.
If Dr. Holtzen thinks an in-person consultation is needed to further assess your needs, the treatment coordinator will make an appointment for you to come into our office. Otherwise, they’ll schedule a time for your procedure and provide detailed instructions and relevant information.
What to Bring
Please bring the following items with you to your first appointment:
- Referral slip
- Current list of medications
- Any recent X-rays or imaging
- Insurance information
- Valid method of payment
- Valid form of identification
Please note: All patients under the age of 18 must be accompanied by a parent or legal guardian.

Financial & Insurance Information

Payment is due at the time of service. Please note that regardless of insurance coverage, any fees charged by our office are your responsibility. If you have any questions, contact our office in Missoula or Hamilton, MT, for prompt assistance.
